Death Certificate

Purpose:

Case specific form that is used to enter information regarding the death certificate.

Navigation Tips:

• Body Release section is view only and is entered on body intake release form
• Place of Death is entered by first entering a type. The type will denote if the user has the ability to enter an address or not. Place Type will show a list of organization types that are defined on form Organization Categories and a list of user defined place types that have no defined agencies. These are defined on form Places with no address on file.
  • Places with address: The user will be able to select an agency from the place field. This will display the address if available.

  

  • Places with no address: The user will be able to enter the specific place and enter the address.

  

• Sub manner of death list is dependent on manner of death field selection
• Death Certificate Amendment will show the latest amendment entered on Death Certificate Amendment.
