Decedent Information

Overall description

Purpose: Form is used to enter Information for the Decedent.
Layout: The page is organized in 7 collapsible sections. See below for specifics if available for each section.

Common Page Functions

Lists and Suggestions:The content of drop down lists, are manneged by the application's administrator.
Any user can suggest a change to the list. (see lists modficiation and suggestions)
Drop Downs and Combo Boxes: User will interact with different controls with arrows. Please see (Drop Downs vs Combo Box)

Section Information

  • Demographic Information
  • Race is a Multi Select drop down see help.

  • Passport
  • Standard data entry.

  • Deceased Address
  • When the check box Same Address as Death Location is checked, all the other fields in the section became read only and they display the information entered for the deceased address.
    By clicking on the Copy Death Location Address all the data from the death location address fields are copied into the deceased address fields and the field remain available to be edited; this could be used as a short cut when most, but not all the information is shared between this two location. After the information is copied over, it can be then modified to the correct values.
    By checking the No Fixed Address - Homeless Transient check box, all the address field are disabled and any current value on those fields will be discarded when the page is saved.

  • Decedent Education
  • Standard data entry.

  • Birth and Death Information
  • Age: System will calculate the age of the decedent if the user enters a birth date and death pronounced date. User can also enter time to calcualte long age (display months days etc). The cutoff for long age display is set by the system administrator.
    User can check Birth Date Unknown, this will disable the birth date, and birth time fields and allow the user to enter the age in Years and Months.
