Request for Documents

Purpose:

This form is used to record requests made for documents or reports.

Navigation Tips:

• Auto-fill of Received Date and Time, Request Name and 1 or many Requested Documents.
• User can check payment waived. If this is checked it will disable the Amount field in the Complete Request Section ADDED TO BILLING HOWEVER WHERE IS AMOUNT FIELD?
• Requested Documents will be an overtype multiple select drop down.
• User will save to complete request
