Identification Naming

Overall description

Purpose: Form is used to change the decedent name, track identification attempts and identification status for a specific case. User can also track different aliases for the decedent
Layout: Form will have two sections: "Identification/Naming" and "Aliases", with a grid on each section.

Section Information

• Identification/ Naming

User Can push +New to bring up popup for entry into the grid Sideways triangle button at top of grid is used to expand all rows in the grid to see additional information. Each row can also be expanded by selecting the sideways triangle for that row. In the popup user has 2 actions they can perform, correct spelling and identification attempts.
Correct Spelling: Selecting this choice allows the user to enter a reason, new prefix, suffix, credentials, last name, first name and/ or middle name.
Identification: Selecting this choice will allow the user to enter a decedent name and inidcate methodology etc in the identification attempt.

• Aliases

User Can push +New to bring up popup for entry into the grid.
User can enter a new aliases into the name field. User can flag 1 aka as primary.
