Medical History

Overall description

Purpose: Form is used to enter medical history of the decedent.
Layout: The page is organized in 4 collapsible sections. See below for specifics if available for each section.

Section Information

  • Medical History Detail
  • User can enter many different blocks of medical history. To bring up pop up user must push +Add new record This will bring up a pop up where the user can choose a type from the drop down, enter content and push checkmark to save. User can enter as many different blocks as there are choices in the history type drop down.

  • Physicians Information
  • User can enter new phsicians by pressing the +New button. This will bring up a pop up where they will be required to enter a physician name. User can also export contents of the grid to excel by pushing button Export to Excel. User can edit rows in the grid by clicking the pencil button at end of row. User can delete a grid row by clicking the x at the end of the row.

  • Hospitalization
  • User can enter new hospitalization by pressing the +New button. This will bring up a pop up where they will be required to enter Name of Hospital. User can also export contents of the grid to excel by pushing button Export to Excel. User can edit rows in the grid by clicking the pencil button at end of row. User can delete a grid row by clicking the x at the end of the row.

  • Surgeries
  • User can enter surgeries by pressing the +New button. This will bring up a pop up where they will be required to enter Procedure Description. User can also export contents of the grid to excel by pushing button Export to Excel. User can edit rows in the grid by clicking the pencil button at end of row. User can delete a grid row by clicking the x at the end of the row.
